ABSTRACT

Objective: To justify the role of open surgical tracheotomy in patients admitted in surgical or medical intensive care units for ventilator support or secretion management


Study Design: A descriptive study


Place and Duration of Study: Department of otorhinolaryngology- Head and Neck Surgery, Dr Ruth K M Pfau Civil Hospital Karachi, Dow Medical College- Dow University of Health Sciences and Otolaryngology-Head and Neck Surgery Ziauddin University Karachi Jan 2014 to Jan 2018


Material and Methods: This study includes all elective and emergency tracheotomies performed in intensive care units (ICU). Patients with cervical spine injuries, bleeding diathesis and patients below 12 years of age were excluded from our study


Results: We encountered complications in 20 patients out of 534, on whom tracheotomies were carried out in intensive care setting. Hemorrhage during and after tracheotomy procedure occurred in eight patients, which was controlled by pressure, ligation or diathermy. Procedure related surgical emphysema occurred in two patients; it was limited to cervical region and subsided with conservative management. The inadvertent decannulation of tracheotomy tube occurred in two cases. Subglottic stenosis developed in four patients. Tracheo esophageal fistula was encountered in four cases


Conclusions: A low morbidity and mortality rate in our series justifies the role of open surgical tracheostomy in patients admitted in surgical or medical ICU for ventilator support or secretion management. It is also effective in a situation of unsecured airway

ABSTRACT

Objectives: To evaluate the awareness and willingness of an eye donation among urban population of Pakistan


Methodology: A cross sectional study was conducted among urban population of Pakistan to access the awareness and concern about eye donation. For this a convenient sampling technique was used and a self-administrative questionnaire was administered. Participants were selected from 3 provinces and 5 major cities of Pakistan. A total of 1520 responded, 52.6% were males and 47.4% were females


Results: 530[66.2%] out of 800 male and 463[64.3%] out of 720 female participants were aware about eye donation. Willingness to donate eyes after death was only seen in about 54/800 [6.7%] males and 58/720 [8%] females. Mass media, television, radio were the most common source of information for both male 342/800 [42.7%] and female 316/720 [43.8%] regarding eye donation. At the end nobility was treated the most common influencing factor for both male 102/800 [12.7%] and female 145/720 [20%] participants


Conclusion: Awareness of eye donation among urban population was not found to be adequate and the willingness for eye donation is too much low. This may be because of lack of high literacy rate in Pakistan, no government eye banks, very few NGO's eye bank and lack of eye donation awareness campaigns

ABSTRACT

Objectives: To determine the outcome and complications of endoscopic dacryocystorhinostomy without stenting at Ziauddin University Hospital, Keamari, Karachi, Pakistan


Methods: Fifty Patients who underwent endoscopic dacryocystorhinostomy from August 2011 to July 2012 at Department of Ophthalmology, Ziauddin University Hospital, Keamari, Karachi were included. Data regarding the Outcomes and complications were collected and analyzed


Results: Among the 50 patients there were 38%[n=19] males and 62%[n=31] females. Their age ranged from 15 to 60 years, mean age was 43.54 +/- 9.36 years. Majority of patients were in the age ranging from 31 to 50 years. The success rate after 6 months of surgery was 92% without any significant complications


Conclusion: Endoscopicdacryocystorhinostomy without stent is minimally invasive technique with less complications and good success state. In addition it gives no facial scar
